TL;DR

Medisafe does more; Pillzy does less, more simply. We make Pillzy, so read this with that in mind. Every fact about Medisafe below comes from its own App Store listing, Google Play listing and help center, checked in October 2026, and we've kept its strengths in. What Medisafe Costs in 2026
Medisafe's price depends on where you live. Its Google Play listing says that in the United States, Medisafe is free to download and use, with Premium as an optional subscription. Outside the United States, it includes a 14-day free trial, and after that you must buy a subscription to keep using the app. Medisafe's help center says this change took effect on January 1, 2026, and lists Premium at $4.99 a month or $39.99 a year (USD). The App Store shows several in-app purchase prices, so check the price in your country before you pay.
According to the listing, Premium includes unlimited medications, unlimited Medfriends, more than 20 health measurements and a dozen reminder sounds. The listing doesn't say how many medications the free US tier allows.
Pillzy is free to download, and its free version covers two medicines or supplements. Tracking more, plus some extras like the PDF report, needs the optional subscription, with the price shown in the app before you buy.
Where Medisafe Is Stronger
Medisafe has features Pillzy doesn't, and for some people they matter more than simplicity.
- Drug interaction warnings. Medisafe checks your medicines against each other for drug-to-drug interactions, though its Play listing says this only works in the US with the app set to English.
- Medfriend. You can add a family member or friend, and Medisafe sends them a push notification if you forget a dose. Pillzy lets a caregiver track someone else's routine on their own phone, but it doesn't alert a second person automatically.
- Health tracking. Medisafe logs blood pressure, blood glucose, weight, pulse and temperature, and syncs with Apple Health.
- Hospital records. In the US, it can import medicines from the health records of more than 100 hospitals and health systems.
- Your wrist. It has Apple Watch and Wear OS apps.
If any of these is the reason you use a reminder app, Medisafe is the better choice.
Where Pillzy Is Stronger
Pillzy is built to do one job: remind you, then record what you took. You add a medicine with its name, dose, instructions and an optional photo, pick a daily, weekly or custom schedule, and choose the times. When the reminder arrives, you mark the dose taken or missed, or snooze it. Everything lands in your history, with statistics and a calendar planner so you can see the gaps.
That narrow focus is the main difference. There are no health measurements to set up, no appointment manager and no second person to invite. For someone who takes a daily multivitamin and one prescription, that's less to configure and less to ignore.
The other difference is availability. Outside the US, Pillzy has a free version you can keep using, while Medisafe stops working without a subscription once the 14-day trial ends. If two items cover your routine, Pillzy costs nothing.
Which App Should You Choose?
Choose Medisafe if
You take several prescriptions and want the app to flag interactions, you're in the US, and you'd like someone to be alerted if you miss a dose. It's also the better pick if you track blood pressure or blood sugar alongside your medicines, or want to log doses from an Apple Watch or Wear OS watch. If you're outside the US, plan on paying for Premium after the trial.
Choose Pillzy if
You want reminders and a reliable record, and nothing else. Pillzy suits people managing one or two daily medicines or supplements, people setting up a routine for a parent or older relative on their own phone, and anyone outside the US who wants to keep a free reminder app after Medisafe's trial ends. If you also take vitamins, Pillzy treats them exactly like prescriptions, and our guide to the best time to take vitamins helps you pick their reminder times.
Consider neither if
You only take one medicine on an iPhone and don't need history or statistics. Apple's Health app has a built-in Medications feature with dose reminders and follow-up alerts, and it's free.
Switching From Medisafe to Pillzy
There's no import between the two apps, so switching means re-entering your medicines. For most routines it takes a few minutes.
- Export your Medisafe history first. Medisafe's help center explains how to download a full history report from the app's support menu. Keep it for your records or your next appointment.
- List what you take. Write down each medicine or supplement, the dose and the exact times, copied from your prescription labels rather than from memory.
- Add each one in Pillzy. Enter the name, dose and instructions, then set the schedule and reminder times.
- Run both apps for a day or two. Check that every reminder arrives at the right time before you turn Medisafe's notifications off.
- Turn off the old reminders. Two apps reminding you about the same pill is how double doses happen.

The Verdict
Medisafe is the more capable app; Pillzy is the simpler one. If you need interaction checks, Medfriend alerts, health measurements or a watch app, choose Medisafe and accept the subscription if you live outside the US. If you need dependable reminders and a clean record of what you took, especially for a short list of medicines and vitamins, Pillzy does that with less setup and a free tier wherever you are.

Frequently asked questions
Can I export my Medisafe history before I stop using it?
Yes. Medisafe's help center says you can open Support and data management in the app and choose Export data to download a report of your full history. The same menu has Delete account if you want your data removed, which can't be undone. Export first, then delete.
Does Medisafe have ads?
Medisafe's Google Play listing is marked "Contains ads". If ads bother you, check what your plan includes in the app before you subscribe, since the listing doesn't say which tiers show them.
Does Pillzy work on Apple Watch or Wear OS?
No. Pillzy runs on iPhone and Android phones, and its reminders arrive as phone notifications. If you want to mark doses from your wrist, Medisafe has an Apple Watch app and a Wear OS app.
Can I use Pillzy for vitamins as well as prescriptions?
Yes. Pillzy treats vitamins and supplements the same way as prescription medicines: each one gets its own schedule, reminder and history. The Pillzy site also has a supplements directory with cited timing notes for common vitamins.
